Storage of Your Information
All the data that I collect from you is stored in my electronic health records (EHR) system called Healthie. Healthie is a company that operates a HIPAA compliant online software for healthcare providers to interact with patients and manage their care. Additionally, I may have some of your information stored within my HIPAA compliant e-mail system called Hushmail. Next, I may have some of your information temporarily saved on my laptop after if I need to scan your healthcare documents to my computer. I will then upload these documents to Healthie before deleting them from my hard drive. Finally, the facility in which I am renting space, TexasMed, may have a digital record of your name and the dates you checked in for your appointments. I have a Business Associate Agreement signed with all three companies, and you can be assured that your information is being safeguarded in compliance with all applicable HIPAA laws.
Transfer of Your Information
I will have an e-fax number associated with my Healthie account for sending and receiving faxes. This will allow me to share your information with other healthcare providers with whom I am collaborating, referring you to, and/or co-managing your care. I may also discuss your case with other healthcare providers over the phone.
Retrieval of Your Information
I will have a record of your health information, including your intake forms, outcome assessment forms, journal entries, and visit notes within Healthie. You will have a profile within Healthie that allows me to search for and retrieve any information I have collected or documented about your case. You may request a copy of your medical records at any time.
Breach of Your Information
If any of your information is disclosed, overheard, leaked, accessed, or compromised in any manner, you will be informed of this in a letter from me or from Healthie (depending on how the breach occurred). Please be assured that every effort will be made to keep your information protected.
Public Display of Your Information
I may ask you for a website testimonial or google review at the end of your treatment plan. This is optional and will not affect any future care you receive. If you choose to write a google review or website testimonial, your name, photo, and any information you volunteer will be publically viewable. To my knowledge, I am unable to remove google review posts, but if at any point you decide you would like to have your testimonial removed from my website, please contact me by e-mail and I will do so. At some point I may have an office picture board of patient “success stories”. If you are willing to have a photo taken to pair with your website testimonial or for the success story board, please know that it will viewable online and by other patients and staff in my office. You can request that I remove the photo at any time.
Other Individuals with Access to Your Information
I currently do not have any employees, interns, or students working in my practice. Thus, no one else will have access to your health information. The staff at TexasMed will have access to your name since you will check-in to see me upon arrival for your appointment, but they will not have access any additional personal or health information. If I hire any employees or volunteers, I will update this document and notify you.
Patient-Provider Communication
There are several means of communication that I may use to deliver and discuss your health information including chat messaging or video calls through Healthie, phone calls, e-mails, and text messages. My personal business cell phone number is 469-343-3441. No one else has access to this phone. If there is a method of healthcare communication in which you do not approve, please specify here.
Website Data
Any information that you submit through the secure contact form on my website is kept within the Hushmail servers and sent directly to my inbox. No third parties will have access to your information. There are several contact points with Healthie on my website. You will be able to login to your Healthie portal, and this would be the same process as going to the following web address: https://secure.gethealthie.com/users/sign_in?target_url=/settings/appointment_types. Furthermore, you can schedule a discovery call with me through a link on my website, but this process will occur entirely through the Healthie platform. None of your information will be viewable or accessible on my website to me or my developers.
Privacy Practices Reassessment
I will regularly review my policies and procedures (at least quarterly) to ensure legal compliance, transparency, clarity, and ease of operation. I will notify you if any changes are made.
